Estrofem is an oral application. While it is still safer than Premarin or Ethinylestradiol, it's still the worst way of hormone replacement therapy in the area of bioidentical hormones. Due to the absorbtion via the digestion system, the liver metabolises about 70% of the effective ingredient. The rest is partially blocked because the metabilization creates estrone which inhibits the access of the estradiol to estrogen receptor alpha. Also, the liver is unnecessarily burdened.
The only way with next to no adverse effects are transdermal applications like gel, spray and patches, or injectable depots into the fat layer under the skin or in a muscle.

Premarin also causes subclinical brain damage. It should be pulled of the shelf. Only patches should be used or sublingual Estrofem. Estrofem is typically swallowed when in fact it should only be kept under the tongue for 30 minutes then spit and rinse. Its far more effective and does not pass through the liver twice thus reducing the risk of DVT and other complications. We as trans women fight for ALL women's right to safer hrt!
