Discovery Health to Review Transgender Healthcare Access After Landmark Member Vote

In a significant development for transgender healthcare in South Africa, members of Discovery Health Medical Scheme have voted in favour of reviewing how the Scheme provides access to healthcare for transgender and gender-diverse members.

Discovery Health Medical Scheme confirmed on 10 July that members had approved the motion, which was tabled and voted on at the Scheme’s Annual General Meeting (AGM) on 25 June.

The decision means that Discovery, South Africa’s largest open medical scheme, will now undertake a comprehensive review of whether transgender and gender-diverse members have fair, reasonable and non-discriminatory access to clinically appropriate healthcare, before reporting its findings back to members.

A Member’s Personal Experience Sparks Change

The motion was submitted by Discovery Health member Brent Janse van Vuuren, a transgender man, after experiencing challenges accessing transgender-related healthcare through the Scheme.

While motivated by his own experiences, Brent said he later discovered that many other transgender South Africans had encountered similar barriers.

The motion did not call for immediate changes to Discovery’s policies or benefits. Instead, it requested an independent review of whether the Scheme’s current benefit design, funding criteria, managed care protocols and decision-making processes align with current clinical evidence, recognised standards of care and South Africa’s constitutional values of equality, dignity and access to healthcare.

The proposed review will also examine whether members receive clear and transparent information about available benefits, funding criteria, review processes and appeals.

A Significant Milestone

Janse van Vuuren described the outcome as an important step forward for inclusive health.

“I’m absolutely thrilled with this outcome. Medical schemes exist to serve all of their members, and that includes their transgender and gender-diverse members.

“This review is an important first step in ensuring that transgender and gender-diverse members are treated fairly when it comes to accessing healthcare,” said Janse van Vuuren.

“Discovery Health Medical Scheme is South Africa’s largest open medical scheme, covering more than 2.7 million beneficiaries, so having members vote in support of this review is incredibly significant.

“As a transgender person, I’ve personally experienced barriers navigating healthcare through the Scheme. My hope is that this review will lead to a better understanding of those experiences and, ultimately, make it easier for transgender and gender-diverse members to access clinically appropriate healthcare in the future,” added Janse van Vuuren.

Review to Examine Policies and Access

Under the approved motion, the Board of Trustees will assess several key areas, including:

  • Whether current benefits and funding criteria for hormone therapy and gender-affirming surgery are adequate, fair and transparent.
  • Whether existing Scheme rules, clinical criteria and managed care processes support or hinder access to appropriate healthcare.
  • Whether current policies reflect recognised South African and international clinical guidance and the latest medical evidence.
  • Whether members receive clear information about available benefits, funding criteria and appeals processes.

While the review does not guarantee changes to benefits or funding policies, it acknowledges concerns raised by transgender and gender-diverse members and creates an opportunity to examine whether current practices deliver equitable access to healthcare.

The Board of Trustees will now conduct the review and report its findings and any recommendations back to Discovery Health Medical Scheme members.
